Description The Social Worker provides services to meet the social and/or emotional needs that affect the residents’ ability to achieve their highest level of function; participate in the development of residents’ comprehensive care plans; develop policies and procedures to provide social services to residents in compliance with federal, state and local regulations. Schedule: As Needed, Part-Time, Weekdays, 4-6 Hours per week Primary Responsibilities and Duties: Develop and implement social service policies and procedures to meet residents’ needs in compliance with federal, state and local regulations. Develop, coordinate and participate in family and resident activities designed to promote social interaction, reality orientation and intellectual stimulation. Develop one-to-one professional relationships with residents and families as needed for counseling. Counsel residents and families in dealing with feelings about death or dying and other emotional, mental environmental or physical limitations. Assess and complete the sections of the (MDS) Minimum Data Set assigned completely, timely and accurately. Document the social service component of the Comprehensive Care Plan for each resident in a timely manner. Document psycho-social needs, interaction and follow-up action(s) with resident and/or family according to facility policies and procedures. Assess, plan and document residents’ discharge needs in accordance with facility discharge planning policy and regulatory requirements. Act as a liaison with social, health and community agencies. Refer residents to social, health and community agencies and complete accurate documentation in residents’ records concerning the results of such referrals. Coordinate family and community resources as needed to solve financial needs and to promote emotional security. Take part in facility in-service programs as required. Requirements Job Qualifications: Bachelors’ degree in Social Work or related field. Current Social Worker license in the state where work is performed. 1-2 years’ experience in social services in Skilled Nursing Facility setting is preferred. Strong understanding of community resources and social service programs. Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, caregivers, and healthcare professionals. Demonstrated ability to prioritize tasks, manage multiple responsibilities, and adapt to changing priorities in a fast-paced environment. Ability to build rapport with patients through a compassionate and caring demeanor. Physical Requirements: Ability to lift and transfer patients, or medical equipment with or without assistance, up to 50lbs only when needed to assist clinical staff. Capability to perform physical tasks such as bending, standing, walking, and climbing stairs for extended periods. Strength to handle medical equipment and assist patients with mobility and physical activities. Visual acuity and dexterity for performing tasks such as reading medical records, administering injections, and handling medical instruments. Ability to respond swiftly and efficiently to emergency situations. Capacity to work in various home environments which may include navigating stairs, tight spaces, or uneven surfaces. Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager. Job requires accepting criticism and dealing calmly and effectively with high-stress situations. The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.
