What We Provide: A positive and supportive workplace that encourages professional growth and development Flexible workdays and hours tailored to fit your schedule Weekends off to support a healthy work-life balance Schedule & Compensation: Position: Full-Time (40 hours/week) Hours: 8:00 AM – 4:30 PM (flexible based on individual needs) Starting Salary: $55,000 annually (commensurate with experience) Job Description Develops and maintains professional relationships with staff, physicians, residents, families, community resources, and third-party payers. Completes Admission forms/assessments and documentation as outlined in the Social Service Admission Process. Completes a plan of care for each resident based on their needs. Documents on progress towards care plan goal, and updates POC with significant change and/or quarterly. Assess residents various health needs on an on-going basis. Assess the need for referrals to health services and provides education and support.
