How you’ll make an impact in this role Psychosocial Assessment: Evaluating complex emotional, psychological, and physical issues to determine the support needed for both patients and their families. Interdisciplinary Care Planning: Collaborating with physicians, nurses, and therapists to develop and reevaluate individualized care plans from admission through discharge. Patient Advocacy: Ensuring the medical system respects patient rights and preferences, especially in difficult cases involving ethics, end-of-life decisions, or abuse/neglect. Resource Coordination: Connecting patients with community resources for financial assistance, housing, food, and long-term care placement (e.g., skilled nursing or rehab). Crisis Intervention: Providing immediate emotional support and counseling for patients and families facing a new diagnosis, grief, or major life changes. What minimum requirements you’ll need Licensure / Certification / Registration: One or more of the following required: Clinical Counselor obtained prior to hire date or job transfer date. Bachelor Social Worker credentialed from the Tennessee Board of Social Workers obtained prior to hire date or job transfer date. Case Manager credentialed from the American Case Management Association (ACMA) preferred. Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. Education: High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
