SUMMARY Under general supervision and the direction of the Program Director or Designee, the Social Worker Assistant provides direct and supportive services to individuals and their families in accordance with ADAPT Community Network policies, the standards of the profession, and all applicable regulatory requirements. This position assists in implementing treatment and service plans, providing counseling and concrete supports, and coordinating access to community and government resources to enhance overall well-being and quality of life. ESSENTIAL DUTIES AND RESPONSIBILITIES The key duties and responsibilities of the Social Worker Assistant include but are not limited to: Provides direct services to individuals and families, as assigned by the Program Director or Certified Social Worker, including assistance with case management, benefits coordination, and service linkage. Supports the implementation of individualized treatment and service plans, offering counseling and practical supports in areas such as housing, food assistance, Medicaid, transportation, and recreational opportunities. Collaborates with families, guardians, and program staff to integrate therapeutic and behavioral interventions into daily routines at home and in program settings. Participates actively in interdisciplinary team meetings, case conferences, and annual reviews to support continuity of care and effective service planning. Documents services provided, maintains accurate case notes, and monitors progress on all assigned cases. Reports significant changes or concerns promptly to the supervising Certified Social Worker. Conducts home visits to provide outreach, assess needs, and maintain positive relationships with families and caregivers. Assists with arrival, dismissal, feeding, and toileting of individuals when necessary to ensure safety and well-being. Participates in departmental and agency-wide in-service training programs to enhance skills and maintain compliance with current practices and regulations. Maintains strict confidentiality of all clinical, programmatic, and family-related information. Keeps informed of developments, resources, and best practices within the field of social services and developmental disabilities. Performs other related duties as requested. QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. High School Diploma or GED required; some college coursework in Social Work, Human Services, or a related field preferred. At least one year of related experience in social services or direct care preferred, particularly within programs serving individuals with developmental disabilities. Ability to read, write, speak, and understand English. Strong interpersonal skills necessary to interact effectively with individuals served, families, coworkers, and community partners. Excellent organizational and time management skills with the ability to manage multiple priorities. Strong oral and written communication skills. • Proficiency in the use of a personal computer and standard office software (e.g., Microsoft Office, Outlook). Ability to work independently, take initiative, and exercise sound judgment. Commitment to maintaining the highest level of confidentiality and professional integrity.
