EDUCATION, TRAINING, AND LICENSURE/CERTIFICATION

  • Associate degree in human services or related field, OR high school diploma/equivalent and 60 college credits or equivalent combination of education and work experience required
  • Bachelor degree in social work, human services or related field preferred

EXPERIENCE

  • One year experience in child welfare or refugee resettlement program preferred

ADDITIONAL WORK REQUIREMENTS

  • Valid unrestricted driver’s license with good driving record with reliable transportation suitable to transport person served. Proof of current automobile registration and insurance
  • On-site, in-person work is required for most scheduled hours
  • Participate in a system of 24 hour on-call availability to house, will be required to be on shift to ensure adequate coverage for compliance

Responsibilities

THIS POSITION WILL PROVIDE SUPPORT AND ASSISTANCE TO THREE MAIN POSITIONS ASSIGNED TO THE UC PROGRAMS: MEDICAL COORDINATOR; INTAKE/DISCHARGE COORDINATOR AND NUTRITION AND FOOD COORDINATOR. THIS MAY INCLUDE COORDINATION OF SERVICES FOR MEDICAL BILLING AND APPOINTMENTS, COMPLETING TASKS INVOLVED IN THE INTAKE AND DISCHARGE PROCESS, COVERING RESPONSIBILITIES FOR OTHER POSITIONS AS ASSIGNED BY THE LEADERSHIP TEAM. RESPONSIBILITY MAY ALSO INCLUDE PROVIDING TRANSPORTATION YOUTH IN THE PROGRAM.

Duties and Responsibilities

  • Provide auxiliary services and direct assistance to clients, under the direction of medical coordinator and /or casework supervisor in accordance with applicable contractual, regulatory requirements and accreditation standards
  • Provide transportation for youth as needed and assigned needed services
  • Supervise children during appointments either at school or in program, in offices, or at other appropriate sites as part of assigned work
  • Observe and document activity related to the child; report observations to social worker with case responsibility or to supervisor as needed
  • Complete progress notes for assigned case responsibilities utilizing computerized software
  • Obtain documentation after attending youth medical appointments for submission to the medical coordinator
  • Assisting the medical coordinator with billing issues and outstanding claims
  • Work alongside the Unaccompanied Children’s Services team to coordinate services for all aspects of the program as assigned by supervisor
