Salary range: £43,299 - £46,512 p.a. inc. London weighting

  • A £5000 ‘golden hello’ for Social Workers new to Brent, upon completion of their probationary period.
  • An annual retention payment of £3000 for eligible frontline workers and managers from Grade PO2 – PO7

Contract: Permanent
Hours of work: 36 hours per week
Location: Civic centre, hospitals and other locations from time to time

The Post

The post holder will work in the evolving Hospital Discharge Team, which supports people who are eligible under the Care Act 2014 to be discharged from hospital due to recent illness, disability, old age and frailty.

The team is responsible for facilitating safe and timely discharge from hospital and works closely with hospital staff to make this happen. We support discharge from hospitals for all Brent residents who need support from social care on discharge from acute, in-patient rehab & older adult mental health wards. Our main acute hospital:

  • Northwick Park, Central Middlesex and Ealing Hospitals
  • Royal Free, Barnet and Chase Farm Hospitals
  • Imperial Group: St Mary’s Hospital, Charing Cross, and Hammersmith
