Overview Social Worker, BSW Marshalltown Hospital Shift: Part-Time | 28 Hours per Week | Monday–Friday | 8:00 AM–4:30 PMIncludes occasional weekend coverage and approximately 1–2 holidays per year. As a Social Worker, BSW, you will provide compassionate support and advocacy for patients and families throughout their healthcare journey. You’ll help identify physical, emotional, mental, and social needs while assisting with care coordination and discharge planning to ensure safe transitions of care. This role serves as a key liaison between patients, families, healthcare teams, and community resources, helping connect individuals with the support services they need both during hospitalization and after discharge. Responsibilities Assess patient and family needs to support safe, effective discharge planning and transitions of care Coordinate post-discharge services and community resources to help patients succeed after hospitalization Collaborate with providers, nursing staff, care managers, and interdisciplinary healthcare teams to support individualized care plans Provide emotional support, counseling, and advocacy for patients and families experiencing psychosocial needs Connect patients and families with appropriate community programs, services, and referral resources Help reduce hospital readmission risk through care coordination and resource management Educate patients, families, and healthcare team members on available support services and care options Maintain accurate and timely documentation within the electronic medical record Communicate discharge planning concerns and patient needs clearly and effectively with the healthcare team Support a collaborative, patient-centered environment focused on quality outcomes and continuity of care Qualifications Education: Bachelor Degree of Social Work (BSW) from an accredited school of Social Work. 1 year of Medical Social Work or related experience preferred License(s)/Certification(s): Valid driver’s license when driving any vehicle for work-related reasons. Possesses an Iowa Social Work License (LBSW)
