JOB DESCRIPTION

The Social Worker provides casework, interventions, and care coordination services to older persons and their caregivers within the home and community settings. The role supports ageing-in-place by addressing the psychosocial, emotional, and environmental needs of clients, facilitating access to care, and working in collaboration with CEF’s multidisciplinary home care team and external partners. This role also supports community development efforts, leads group interventions, and participates in research and service improvement initiatives.

EDUCATION & PROFESSIONAL CERTIFICATION

• Degree in Social Work or equivalent recognised by the Social Work Accreditation and Advisory Board (SWAAB).
• At least 3-5 years of experience in medical social work within an acute or community care setting.
• Prior exposure to eldercare or services for vulnerable adults and community care landscape is an advantage.
• Experience working with clients with dementia, frailty, or palliative care needs is an advantage.
