Join our team as a day shift, full-time, Case Management Acute Care Social Worker (MSW) in Tulsa, OK.

  • Hillcrest Medical Center is a 656-bed hospital that includes The Alexander Burn Center, The Peggy V. Helmerich Women’s Health Center, Kaiser Rehabilitation Center, the latest technology for the treatment of cancer, and the Oklahoma Heart Institute.

Responsibilities:

  • Act as liaison between the patient and medical personnel
  • Work within behavioral health to complete comprehensive mental health exams and psych patient assessments
  • Facilitate long term health and well-being through counseling, education, and brokering of needed community services and resources
  • Develop and implement appropriate treatment plans for each patient
  • Serve as an advocate for patients and their families in regard to decisions affecting plan of care
  • Act as a resource for hospice and counseling services
  • Provide appropriate end of life planning and care
  • Ensure timely discharge planning

Qualifications:

Job Requirements

  • Master’s of Social Work degree

  • Previous hospital experience
