The Drug Health Social Worker provides assessment and care co- ordination of clients, liaison with families and other services to facilitate drug and alcohol treatment within a harm reduction framework. As part of the multidisciplinary Centre for Addiction Medicine Team, the position operates within a harm reduction framework and also acts as a resource supporting colleagues to effectively utalise community health and welfare services and implement care plans for clients with highly complex health and psychosocial needs.
