ABOUT THE ROLE..

As a Social Worker and Case Manager in our Brain Injury Unit (BIU), you’ll play a vital role in supporting clients through their recovery journey. You’ll provide evidence-based clinical care, guide rehabilitation case management, and ensure smooth and well-supported discharges back into the community.

KEY RESPONSIBILITIES INCLUDE:

  • Counselling to assist clients and their family/carers to deal with the emotional impact of injury, illness and disability.
  • To undertake case planning, liaison, advocacy and referral to maximise access to appropriate community services.
  • Liaise with multi-disciplinary team colleagues to optimise client care.
  • Contribute to the ongoing development of the social work and case management services.
