Responsibilities include assessing psychosocial needs, identifying barriers to discharge, coordinating community resources, managing patient cases, and maintaining timely documentation in the electronic medical record. The Social Worker Case Manager works alongside Social Service Specialists to support resource coordination and non-clinical discharge tasks, but in collaboration with RN Case Managers, they lead the management of complex discharge needs requiring clinical judgment. They also facilitate safe and appropriate transfers to the next level of care; including skilled nursing facilities, inpatient rehabilitation, and long-term care, and arrange for post-acute services such as home health and hospice. This role is integral to ensuring timely, patient-centered discharge planning and continuity of care. Qualifications What is required... Masters of Social Work (MSW) degree from an educational program accredited by the Council on Social Work Education (CSWE) BLS/CPR Certification within 90 days of hire At least one year of previous hospital case management experience What is preferred... Hospital inpatient case management experience Case Management Certification Rural healthcare experience Experience with Cerner EHR Additional Information Wage: Wage range starts at $31.66/hour and goes up to $47.99/hour.
