Description

CentraCare’s Plaza expansion project is well underway and will be completed soon! We are looking to grow our team and add a dedicated Social Worker to support patients primarily within our Outpatient Rehab Clinic as well as providing additional support to primary care services across the St. Cloud area. As a Social Worker, you will play an important role in facilitating patient-centered care through a collaborative and comprehensive approach. Your work will include assessments, facilitating and advocating for options and services to meet individual health needs, discharge planning and much more! Schedule Full-time | 80 hours every two weeks | Hybrid position Monday - Friday | 8-hour shifts between 7:30am - 5:00pm Pay & Benefits Pay based on experience and degree Bachelor’s in Social Work starting at $32.09 per hour Master’s in Social Work starting at $33.15 per hour Pay range: $30.65 - $49.73 per hour $3,000 hiring bonus and $5,000 relocation bonus for eligible candidates! Qualifications Bachelor’s or Master's degree in Social Work Current Licensed Social Worker (LSW) in Minnesota A Valid Minnesota driver's license, a reliable vehicle available for work use, a driving record in good standing and the minimum amount of insurance liability coverage required by CentraCare To be eligible for the hiring and relocation bonus you must be an external candidate who is hired into an eligible full or part-time benefited position. Previous employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire. To be eligible for the relocation bonus, you must currently reside more than 50 miles away from location you’re hired to work and relocate to within 30 miles of it within 120 days of employment.
