Description

In keeping with the Mission, Vision and Values and to utilize Human Resources, the Social Worker is responsible for the provision of social work services to clients and their families. The incumbent also provides feedback and direction to staff related to behavioral issues, completes psychosocial assessments of patients, participates in team functions and schedules all meetings involved with program and clients. As a member of the interdisciplinary team, the Social Worker provides client-centered care through the provision of psycho-social assessments; individual, family and potential group counseling. Knowledge of the interaction between physical, emotional and mental health and demonstrating the skills to provide clinical care to individuals and families experiencing difficulties in these areas is essential.

KNOWLEDGE, SKILL AND EXPERIENCE

Formal Education
o Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker in good standing
o Master’s degree in Social Work from an accredited School of Social Work preferred
o Recent (3-5 years) experience providing individual, client and family support services
o Demonstrated knowledge of behavioral and suicidal risk assessment, crisis management, and verbal diffusion techniques
o Working knowledge of the Mental Health Act, Health Care Consent Act, Substitute Decisions Act, and community mental health resources
o Demonstrated skills in the performance of a therapeutic role including strong assessment, counseling, case management and advocacy skills
o Strong interpersonal, team work, and communication skills
o Evidence of commitment to ongoing continuing education of self and others
Other Knowledge, Skill and Experience
o Knowledge of Specialized programming for care and therapy in community
o Knowledge of Community Services and referrals processes
o Knowledge of current development and research in Gerontology
o Knowledge and skill in dealing with seniors, compromised individuals, high risk populations
o Knowledge and skill in group, individual and family therapy
o Demonstrated Leadership ability

Responsibilities
  1. Performs all functions in accordance with the Mission, Vision and Values and Administrative policies and procedures of St. Joseph’s Villa care programs.
  2. Must be able to consult with nurse practitioner and other allied health professionals as part of a multidisciplinary team.
  3. Experience with the ability to triage client needs, manage a waitlist and refer to other providers and programs is required.
  4. Maintain community and mental health resources and connections for clients, networking with community social workers and programs to develop partnerships that support clients.
  5. Working in this environment requires health teaching to clients, families, and volunteers, as well as collaboration with community resources in providing services and education to client’s and their families.
  6. Connect family members with community resources to support their personal needs and required supports.
  7. Provides goal-based counselling to clients who are cognitively capable of participating in the process.
  8. Participation in interdisciplinary client conferences as needed.
  9. Provides clinical services to clients and their families/ significant others as required or requested. This includes but is not limited to biopsychosocial assessments, counselling, problem solving and making connections for future services within community and development of client safety plans.
  10. Reports concerns and complaints and participates with complaint investigations, as required with the nurse manager.
  11. Participates in coordination of bed offers where transition to LTC maybe challenging for the client.
  12. Serves as a community liaison through referral(s) to alternate therapy, programs or supports in the community.
  13. Facilitates meetings with care team members, clients, families and OHAH Care Coordinators when needed.
  14. Participates in committee, in-service education and other CALTC program activities.
  15. Participates in department team meetings and actively contribute to the development of a dynamic, knowledgeable and cohesive team.
  16. Maintain and hold in strict confidence all records and information relevant to the client.
  17. Conduct his/her professional practice in accordance with the Code of Ethics for Social Workers, approved by the C.A.S.W. Board of Directors January 1994 and remain registered with O.C.S.W. S.S.W.
