Providing compassionate care for those living with an illness while supporting those caring for a loved one is the mission of Community Healthcare of Texas POSITION SUMMARY To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patient’s needs/wishes with team members, family members, caregivers, and other providers. ESSENTIAL FUNCTIONS * Assess the psychosocial needs of the patient and family members * Provide therapeutic counseling and emotional support to patients and family members and education on end of life issues * Contribute to and execute the Plan of Care with family/patient * Record daily documentation of visits and contact in accordance with Policy * Provides referrals to community resources * Performs in-office/non-clinical administrative duties * Perform other duties as assigned POSITION QUALIFICATIONS * Bachelors of Social Work with 1 year of applicable experience required; Master's degree preferred * Texas State license required * Reliable transportation and valid driver's license. * Hospice or healthcare experience preferred. * Proficient skill using a computer is required Reasonable Accommodations Statement To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Responsibilities
