Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them. Community Healthcare of Texas is currently recruiting for a Social Worker. POSITION SUMMARY To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patients’ needs/wishes with team members, family members, caregivers, and other providers. ESSENTIAL FUNCTIONS * Assess the psychosocial needs of the patient and family members * Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues * Contribute to and execute the Plan of Care with the family/patient * Record daily documentation of visits and contact in accordance with Policy * Provides referrals to community resources * Performs in-office/non-clinical administrative duties * Performs other duties as assigned POSITION QUALIFICATIONS * Bachelor of Social Work with 1 year of applicable experience required; Master's degree preferred * Texas State license required * Reliable transportation and a valid driver's license. * Hospice or healthcare experience preferred. * Proficient skills using a computer are required Reasonable Accommodations Statement To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
