Community Healthcare of Texas has provided hospice and palliative care services since 1996.  Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them. Community Healthcare of Texas is currently recruiting for a Social Worker.   POSITION SUMMARY To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patients’ needs/wishes with team members, family members, caregivers, and other providers.   ESSENTIAL FUNCTIONS    * Assess the psychosocial needs of the patient and family members * Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues  * Contribute to and execute the Plan of Care with the family/patient   * Record daily documentation of visits and contact in accordance with Policy   * Provides referrals to community resources   * Performs in-office/non-clinical administrative duties   * Performs other duties as assigned   POSITION QUALIFICATIONS * Bachelor of Social Work with 1 year of applicable experience required; Master's degree preferred * Texas State license required * Reliable transportation and a valid driver's license. * Hospice or healthcare experience preferred. * Proficient skills using a computer are required Reasonable Accommodations Statement  To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
