Responsibilities
  • Raise awareness of mental health and dementia through engaging outreach activities and events.
  • Identify and assess individuals at risk, providing timely intervention and support.
  • Coordinate care by linking clients to appropriate health and social services.
  • Offer emotional support to clients and caregivers, maintaining clear and accurate case documentation.
  • Plan and run programmes such as cognitive stimulation, wellness activities, and caregiver support groups.
  • Build networks with community partners and stakeholders to strengthen mental wellness outreach.
  • Manage data and records in line with CREST and PDPA guidelines.
