As a Social Worker at Crossing Rivers Health, you will report to the Inpatient Services Director and provide comprehensive psychosocial support, care coordination, and resource guidance to individuals, families, and groups coping with acute, chronic, or terminal health conditions. Working with the various clinical areas through the organization, this role involves assessing social, emotional, and environmental factors that influence health outcomes and access to care, while delivering supportive interventions, education, and counseling to patients and caregivers. Working collaboratively with the interdisciplinary care team, the Social Worker addresses social determinants of health, assists with care planning and transitions, and supports patient and family decision-making. The position also includes case management responsibilities, coordination of services, and referrals to community resources to promote recovery, prevent illness, reduce barriers to care, and enhance overall stability and well-being.
