This is a full-time, permanent role with hybrid working, and a salary of £38,192 to £42,012 per annum plus an annual retention bonus of £3,000 subject to eligibility.

Responsibilities

THE ROLE:

You will be working as a Social Worker to support young people in transition from Children’s to Adult Learning Disability Services.

YOUR ROLE WILL INVOLVE:

  • Located within a multi-disciplinary community team you would be supporting young people with learning disabilities and their families and carers to help ensure a positive transfer to adult services, including liaising with Children’s services and any other involved agencies.
  • You would be involved in carrying out assessments, undertaking care reviews and supporting care planning.
  • You would take part in a duty rota to deal with incidents, safeguarding alerts and information requests.
  • You would work as a Social Worker and Health Professionals to protect and promote the interests of vulnerable people.
  • Undertake safeguarding investigations and capacity assessments and best interest decisions relating to adults with a learning disability.
