Working collaboratively with a number of MHAS providers and the community, the primary responsibility of the incumbent will be to provide assertive outreach case management services in a variety of community settings to a wide range of adult clients. The incumbent will work as part of a team focused on providing psychosocial case management supports to persons in need of assistance with understanding their illness, recovery planning, housing, ongoing engagement and assessment, coordination of services and supports and managing crisis as relevant. Depending on client demand the incumbent may be assigned to provide counseling supports to other MHAS Teams as required.
