The core functions of case management practice include referral/intake, engagement, assessment, care planning, monitoring, evaluation, termination/discharge, advocacy and coordination of resources through collaboration with the family, care givers, and other members of the multi-disciplinary team. The Social Worker liaises with other services such as acute care, rehabilitation, home care, long term care, mental health and an array of government and non-government agencies in the Regina area and throughout the province, to provide client-centered, strength-based, flexible, cost-conscious and quality driven service at the right time and place.
