If you thrive on challenge and are ready to make a positive and rewarding change to your career, then we would welcome your application.  We have a full-time vacancy for a Social Worker within the Eastbourne & Conquest Hospital Community Team, which is based at St Mary’s House in Eastbourne.  As a Social Worker in Adult Social Care, you will play a key role in the assessment of adults who have recently been discharged from hospital and will support them with their ongoing care planning in order to maximise their independence. You will hold detailed knowledge of the Care Act (2014), Mental Capacity Act (2005), Equality legislation and the Deprivation of Liberty safeguards, and will implement this in your daily practice. You will also be expected to carry out the Local Authorities safeguarding duties to protect and mitigate risks to the individuals we serve.  We have excellent relationships with local partners and hospital trusts, and you will have the opportunity to work alongside other professionals to ensure good outcomes for the individual’s you are working with. Where necessary, we work with our local Integrated Commissioning Boards (ICB’s) to ensure people we serve get access to continuing care funding.
