JOIN OUR COMPASSIONATE TEAM AT ST. PAUL’S HOSPITAL

We are seeking a dedicated and compassionate Social Worker to join our dynamic team in the Emergency Department at St. Paul’s Hospital. In this role, you will help local community members navigate emergent and urgent social/emotional crises related to acute health needs that bring them to the hospital, in a fast-paced, dynamic healthcare environment. You will support patients and families in navigating crises, accessing primary care, social services, benefits, and community supports. Additionally, you will provide critical support to patients and families at various points in their healthcare journeys, starting from their initial presentation at the hospital.

EDUCATION

  • Master’s in Social Work from an accredited university, with one year of recent related experience in a designated program area, or an equivalent combination of education, training, and experience.
  • Current full registration with the British Columbia College of Social Workers.
  • A Bachelor of Social Work degree from an accredited School of Social Work, along with an equivalent combination of education, training, and experience, may also be considered.

SKILLS AND ABILITIES

  • Ability to effectively communicate with patients, families, staff, physicians, and healthcare providers.
  • Knowledge of healthcare legislation, policies, and procedures.
  • Skilled in gathering data and conducting biopsychosocial assessments.
  • Experience in patient/resident assessments and understanding their needs.
  • Knowledge of other healthcare disciplines and their roles in care.
  • Ability to explore issues beyond initial concerns to understand patient/resident situations.
  • Strong collaboration skills within interdisciplinary teams.
  • Experience in conflict resolution and guiding others through it.
  • Physical ability to perform position duties.
  • Capable of working independently and managing priorities.
  • Proficient in operating related equipment.
  • Ability to participate in research and contribute to teaching and supervision of students.
  • Skilled in facilitating group and inter-group meetings, processes, and decision-making.

Responsibilities
  • Supporting the interdisciplinary team in achieving patient and resident goals under the direction of the Patient Care Manager.
  • Providing therapeutic clinical counseling services to patients and residents.
  • Conducting comprehensive biopsychosocial assessments to evaluate needs.
  • Maintaining accurate patient/resident records to ensure continuity of care.
  • Providing consultative and direct services regarding community resources to patients, families, staff, and community professionals.
  • Leading discharge planning with the interdisciplinary team, ensuring smooth transitions for patients and residents.
  • Coordinating clinical/case conferences, ensuring effective communication and decision-making regarding patient care.
  • Developing and delivering in-service training for staff to enhance team capabilities and knowledge.
  • Participating in committees and meetings, contributing to program development and improvement.
  • Staying informed about advancements in the Social Work profession, ensuring up-to-date practices and knowledge.
