RESPONSIBILITIES * The Social Worker will educate the clients and their families about how to manage a severe mental illness and meet their individual goals.  * Conduct daily home visits using the agency vehicle. * The Social Worker will create individual treatment plans based on the clients’ needs. * Complete client assessments, relapse prevention plans, and housing applications.  * Develop individual goals to work toward including a stable plan for medication, housing, and employment for each client.  * The Social Worker will provide evidenced based practice treatment to clients with a severe and persistent mental illness. * Connect clients to treatment facilities including our clinics and other mental health resources in the community.  * The Social Worker will perform crisis intervention by using the crisis phone after hours and on weekends, as needed.  * Conduct group meetings with clients and their families to help rebuild trust and a relationship.  * Travel is required using the agency vehicle.  * All other duties as assigned. QUALIFICATIONS Master’s Degree required. CASAC, LMSW, LCSW, LMHC preferred. 1 year of experience working with the severe and persistent mental illness (SPMI) population required. Ability to work independently and as part of a multi-disciplinary team required. Excellent interpersonal and communication skills, both verbal and written required. Proficient computer skills, including Microsoft Office and Electronic Health Records/Documentation, required. Ability to multitask and prioritize in a fast-paced environment. Valid and clean New York State Driver’s License required.   PHYSICAL REQUIREMENTS  Sitting and standing for extended periods of time, as well as moderate lifting.  Must be able to go up and down stairs.
