JOB DESCRIPTION:

Liquid Personnel is hiring a Social Worker for its client’s Front Door Team in Redbridge.
The role involves working as a care management social worker to assess the needs of vulnerable adults and their carers, and to arrange services that meet identified needs and outcomes. Promoting prevention, independence, self-directed support, choice, and control is central to the role. Key responsibilities also include risk management, safeguarding, and providing case management for adults requiring frequent specialist intervention.

QUALIFICATIONS & EXPERIENCE:

  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK driving licence
  • Post-qualified experience
  • Degree level or equivalent in Social Work
Responsibilities

WHAT WILL YOUR RESPONSIBILITIES BE?

  • Conduct timely and proportionate initial assessments of service users and their carers to identify their needs and desired outcomes.
  • Provide immediate support and intervention to adults in crisis to stabilize their situation and prevent unnecessary hospital admissions.
  • Develop and implement individualized care plans that promote independence and recovery, linking service users to community-based services that enhance personal wellbeing.
  • Work as part of multi-agency teams to provide seamless health and social care services, ensuring effective communication and coordination.
  • Involve service users and carers in creative support planning, encouraging choice and control through the use of personal budgets, including Direct Payments as the default position.
  • Collaborate with Community Development Staff to help service users access community-based services and resources.
  • Adhere to risk management and adult safeguarding procedures in social work practice.
  • Conduct assessments and reviews of service users to a high standard, ensuring they reflect the needs and outcomes of the service user and carer.
