Job Description: Social Worker Grade 2 Health Independence Program - Community Rehabilitation Friendly and supportive team environment Part time, 16 hours per week, Kew location Fixed term until 30 June 2026 About the Role An exciting opportunity currently exists for a Grade 2 Social Worker to join the Community Rehabilitation team on a fixed term part time basis. You will work collaboratively with clients and carers to support and empower them to remain as independent as they can and remain socially active within their communities. Provide time-limited, goal–orientated care for people who have a physical disability, are frail, chronically ill or recovering from traumatic injury or illness to regain and /or maintain optimal function Link clients and carers in with appropriate services and community supports to enhance their outcomes. Deliver services in the Community Rehabilitation Centre, the community setting and the client's home. Classifications for this position will range from Social Worker Grade 2 SC21 - SC24 ($90,235 - $103,724 per annum pro rata). HIP consists of integrated hub sites across several locations including in Fitzroy, Kew, North Richmond and Fairfield. This position may be required to work at any of the HIP hub sites or programs. This will be at the discretion of the HIP manager and will take into consideration appropriate skills and training. Community Rehabilitation services provide time-limited, goal–orientated care for people who have a physical disability, are frail, chronically ill or recovering from traumatic injury or illness to regain and /or maintain optimal function. Community Rehabilitation services are comprised of Community Rehabilitation Centres at Fairfield and Kew including the cardiopulmonary rehabilitation and Rehabilitation in the Home. This position is predominantly situated within the Community Rehabilitation Service across Kew and Fairfield sites, and may include telehealth and home or community visits, or may be required to consult on patients from other HIP services. . POSITION PURPOSE To work within a flexible, interdisciplinary structure, that ensures the delivery of high quality rehabilitation services to all clients of HIP services, making sure that the client’s needs are met and they receive ‘the right treatment in the right place at the right time’. To provide clinical and service leadership in relation to Social Work within Community Rehabilitation, including leading service improvement changes. Clinicians are required to provide assessment and management of clients in a number of settings across the continuum of care within HIP, including client’s residences, community settings, the HIP Centres or Specialist Clinics POSITION DUTIES Assess and manage all assigned clients, including completion of comprehensive psychosocial assessments, according to HIP and St. Vincent’s policies and procedures and relevant rehabilitation protocols Deliver client focussed, high quality social work assessment, intervention, education and management Actively identify and advocate for social work intervention in clients of community rehabilitation and educate other clinicians on the role and scope of social work within community rehabilitation Provide collaborative care coordination for clients and carers as required, in the context of their health, functional, psychosocial, cultural, environmental and economic needs. Effective negotiation and advocacy with team members, community professionals, relevant service providers and other key stakeholders to ensure optimal and coordinated client care according to best practice guidelines Lead service improvement initiatives relevant to the HIP/ Community Rehabilitation social work service Communicate client issues effectively with other team members, with active contribution to multidisciplinary team meetings; identifying supports that can be offered at case review. Complete client goal setting in consultation with patient/family/carers. Goals are to be functional, measurable and based on addressing health and wellbeing needs. Evaluate treatment / management using the most appropriate methods, making use of objective measures where possible Provide education sessions and support for clients of the cardiopulmonary programs Maintain current knowledge and skills in clinical best practice and rehabilitation Participate in relevant in-service programs Meet activity requirements as agreed with Team Leader Maintain accurate statistics and submit on the same day Maintain the appropriate standard of medical record keeping and data collection as per St. Vincent’s and HIP policies Social work service provision to other programs within the Rehabilitation & Clinics Stream and HIP as required Supervision of students as required SELECTION CRITERIA ESSENTIAL REGISTRATION, LICENSE OR QUALIFICATION REQUIREMENTS Formal Education: Bachelor of Social Work Degree or equivalent Relevent experience, preferably in a rehabilitation or hospital setting Eligible for Registration with Australian Association of Social Workers (AASW) OTHER ESSENTIAL REQUIREMENTS A demonstrated sound knowledge and application of social work theory, practice and principles in the areas of rehabilitation and/or community care.
