Job Description: Friendly and dynamic team Permanent / Full Time position based in the Rehabilitation Unit at St George’s Hospital Kew No after hours or on call roster About the Role An opportunity currently exists for an experienced Grade 2 Social Worker to join our team on a permanent basis. At St Vincent’s we have an approach to working with patients which puts them at the heart of their care. This position will work with patients admitted to the Rehabilitation Unit at St George’s Kew. To be successful in this position, it is essential that you have a number of years of experience working in an inpatient hospital social work position. Previous work with patients in a rehab setting or who are on the NDIS will be highly regarded. You will be required to: Use your skills to undertake multi-dimensional, comprehensive psychosocial assessments, apply clinical reasoning, and formulate appropriate complex intervention and discharge plans Liaise with patients, their families and/or carers to co-ordinate, facilitate and communicate patient treatment and outcomes Actively participate in quality improvement and research activities Provide supervision and participate in clinical education programs as required Promote and develop positive relationships within the department, wider organisation and with community partners Classifications for Grade 2 Social Workers range from SC21 - SC24 ($90,235 - $103,724 per annum).
