About Department The Social Work Department at SVHM is recognised as a leader in developing and delivering best clinical practice to highly vulnerable groups and people experiencing all forms of minority stress as well as a widely respected Aboriginal Health Unit. The successful candidate will be provided with a comprehensive and flexible professional development program, as well as structured supervision and support for clinical decision making. With a strong focus on innovative service delivery and continuous practice improvement, you will also benefit from the knowledge and support of a senior team who is dedicated to research and quality improvement. Classifications for this position will range from Social Worker Grade 3 SC31 – SC34 $104,135 - $118,981 per annum, pro rata for part time). About You You will have highly developed skills in assessment and complex discharge planning, as well as the ability to work autonomously and as part of a multi-disciplinary team. You will contribute to positive outcomes for patients via the provision of high quality social work service, using proficient clinical skills, effective communication and collaboration. Undertake comprehensive health screening, assessment, treatment, management and discharge planning of allocated patients Promote and foster clinical excellence and high standards of social work service delivery Participate in multidisciplinary meetings, case conferences, ward rounds and other clinical management meetings for team based care planning.
