QUALIFICATIONS:

  • Masters’ degree in Social Work (MSW or MSSW) from an accredited school
  • Georgia LMSW or LCSW; maintain license as required
  • A minimum of two years of chronic disease case management or social work in a medical setting.
  • Solid knowledge of social work practice: strong assessment, interview and counseling skills.
  • Self-directed; ability to work independently; ability to multi-task
  • Ability to prioritize work assignments and manage time efficiently
  • Proven ability to work effectively in a team environment
  • Ability to work without supervision, to use independent judgment, and to make appropriate decisions
  • High level of interpersonal skills to handle sensitive and confidential situations. Position continually requires demonstrated poise, tact and diplomacy.
  • Ability to maintain strict confidentiality of patient information
  • Provide own vehicle, drive, and be willing to travel as required; including flexible work hours to include evenings and weekends.

COUNSELING AND PSYCHOEDUCATION

  • Provide client with education regarding bleeding disorders and reinforce education given by other team members.
  • Provide education and counseling regarding children’s age-appropriate developmental processes, and normalization of client’s family life while adjusting to chronic illness.
  • Provide vocational counseling to clients with special attention given to exploring health insurance options.
  • Assist client and family with transition from pediatric to adult clinic.
  • Provide support and counseling to clients and families to help them deal with bleeding disorders and their complications.
  • Assist families and significant others with bereavement and loss issues.
  • Facilitate support/peer groups as required by the Director of Clinical Services.
  • Assess and refer clients and their families to mental health services when appropriate.
  • Participate in HoG client activities and events as directed by the Director of Clinical Services.
  • Develop psychosocial sessions for HoG-sponsored client events.
  • Participate in social work supervision with the Director of Social Work as required by licensure rules.
  • Be successfully checked at least twice a year by the Director of Social Work on social work skills.
  • Develop a resource network of agencies and/or groups that provide services needed by HoG clients. Research these agencies and/or groups to determine if the services provided are relevant and appropriate for HoG clients, and to review their eligibility criteria. Document unmet needs.
  • Provide education and application assistance regarding HoG services, programs, and activities including the Bleeding Disorder Assistance Program, Client Assistance Program, Emergency Assistance program, pharmaceutical patient assistance programs, and other HoG programs.
  • Develop and maintain a sound knowledge base in all facets of hemophilia and other bleeding disorders, the nature of the disorders, treatment, complications and their treatments, client population, aspects of Hemophilia of Georgia, Georgia resources, and third-party payers.
  • Assist with program planning and evaluation.
  • Conduct outreach to relevant community systems, such as schools, hospitals and clinics, to provide education regarding psychosocial aspects of hemophilia and HIV.
  • Articulate client concerns when serving on community forums or task forces.
  • Provide training and consultation to other social work professionals, social work students/interns and other professionals working with people with a bleeding disorder.

  • Seek training sessions and workshops to continually improve skills.
  • Remain informed of research and other resources for client education and referral regarding issues of concern to people with bleeding disorders.
  • Compile data for statistical purposes as directed by the Director of Clinical Services.
  • Participate in psychosocial research as approved by the Director of Clinical Services.
  • Research those clients who are lost to follow-up (i.e., not seen in recent years or those with no current address in the HoG database).
