Responsibilities Comply with established Social Work standards of care. Effectively educate patient/family and community regarding the Hospice philosophy, concept and HFC services. Conduct a thorough assessment each visit, identifying patient/family needs/problems/concerns and effectiveness of current SW interventions. Assist team members in developing an Interdisciplinary Care Plan. Care plan includes all problems identified by SW assessment, realistic goals and interventions, patient/family input and frequency of visit. The SW will review and revise the care plan biweekly. Provide team with psychosocial indicators that may be pertinent to the plan. Assist team in revising the plan as indicated Enhance patient/family ability to cope. Communicate openly and meet care giving needs by providing effective counseling techniques, family conferences, and education. SW frequency of visit will be increased or decreased appropriately as indicated by patient/family needs. Remain current regarding available resources, eligibility requirements and application process for access to them. Completes financial assessment and applies for resources where indicated. Remain current regarding Advanced Directives. Educate patient/family regarding these directives and assists with completion where indicated. Remain current regarding Medicare, Medicaid and private insurance benefits. Educate patient/families regarding coverage and how to maximize benefits. Assist team in assessing patient eligibility for Hospice services. Provide psychosocial information that may be pertinent to the decision. Assist with discharge planning, identifying and accessing services available as agreed upon by patient/family. Complete necessary documentation. Following the death, complete work in progress (obtaining financial support, arranging for resources, supportive counseling) and follow up with bereavement note. Demonstrate ability to plan and prioritize based on patient/family needs, caseload constraints and expectations of the Agency. Submit weekly schedule of visits to supervisor. Assists with admissions PRN. Performs other duties as assigned. Qualifications Master's degree preferred in Social Work from an Accredited School of Social Work. Current Social Work License from the State of Alabama or prepared to take exam. One-year postgraduate experience. Experience in health care. Must have an automobile in good working condition, automobile insurance, and a current Driver’s License.
