KeystoneCare is seeking a compassionate and dedicated Social Worker to join our Hospice-Palliative Care team in providing essential support to patients and their families. As a key member of the interdisciplinary team, you will assess the psychosocial needs of patients and their families, providing counseling, crisis intervention, and resources to help manage the emotional and practical challenges associated with terminal illness. Provide emotional support, crisis intervention, and counseling to patients and families. Assist in developing and implementing individualized care plans. Connect patients and families with community resources and support services. Participate in interdisciplinary team meetings to ensure comprehensive patient care. Document assessments, interventions, and outcomes in accordance with agency policies and regulations. Educate patients and families about advanced care planning and end-of-life issues. Master's Degree in Social Work from an accredited program. Current Pennsylvania Social Work license (LSW or LCSW). Experience in hospice, palliative care, or home health preferred. Strong understanding of psychosocial aspects of end-of-life care. Excellent communication and interpersonal skills. Ability to work effectively as part of an interdisciplinary team. Knowledge of community resources and healthcare systems. Valid driver's license and reliable transportation. Proof of COVID-19 Vaccination or an approved Medical or Religious exemption. Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption.
