Your Opportunity:
The Youth Addiction and Forensic Mental Health portfolio has an exciting opportunity for an experienced Social Worker. The Social Worker II provides a range of generalist Social Work services to children, youth and families within the PChAD Program and our voluntary inpatient treatment programs. The PChAD Program is a court-ordered detoxification and assessment program for youth aged 12-17 years, harmfully involved in substance use. Youth are confined in a Protective Safe House for a period of 10 days to safely withdraw from substances, undergo a comprehensive assessment of their use, and receive recommendations on voluntary options post-discharge. As a Social Worker II, you will be responsible for case management, transition planning, psychosocial assessment, resource-based interventions, risk screening and group delivery. This position requires skills in engaging adolescents experiencing challenges with substance misuse and abuse, mental health issues and complex family systems. The position involves consultation and collaboration with interprofessional team members, stakeholders, and community partners and requires the ability to work independently with occasional review, consultation and evaluation from a manager. To be successful in this role, you will have strong organization skills and thrive on being self-directed in your work. You will be comfortable navigating the complexities of working across different systems and working with complex issues relating to the addiction and mental health of children, youth and families.
