Your Opportunity:
Reporting to the unit manager, the Social Worker II (SWII) works as a member of a health care team, collaborating on the delivery of care to meet the needs of patients. The SWII is responsible and accountable to the public for his/her own practice, according to the standards defined by the Alberta College of Social Workers (ACSW). The SW II will provide a range of social work services to patients and their families in an acute care setting. Working within the context of patient and family-centered care and multidisciplinary service delivery, the SWII will collaborate with a range of other service providers within AHS and community partners to provide support and clinical intervention to patients on the in-patient mental health Short Stay unit. The primary focus of the SWII will be centered on discharge planning and placement to ensure seamless patient care and transition back into the community upon discharge. The SWII will assess, plan and implement care of patients and work in consultation with the patient’s treatment team and other community providers to meet the needs /complex needs of the patients following the principles of social determinants of health. The SWII will have ongoing assessment and evaluation of real or potential risks to the patient and family and recommend interventions to mitigate those risks.
