Job Title: Social Worker
Status: Regular Part Time
Department: Social Work: PASS
Program: Healthy Aging
Days and Weekends (alternating weekends required) - 8 Hour Shifts (Subject to Change)
Location: Lakeridge Health Ajax-Pickering (primary) & Oshawa
Wage Schedule: $44.670 - $57.900 per hour

QUALIFICATIONS:

  • Master of Social Work Degree; registration, in good standing, with the Ontario College of Social Workers and Social Service Workers;
  • Experience with inpatient mental health and substance use populations, and provision of care through a range of modalities;
  • Demonstrated skill in mental health assessment, crisis management and knowledge of applicable legislation;
  • Demonstrated experience in working collaboratively with inter-professional teams and community support agencies;
  • Experience in delivering comprehensive social work interventions within Medicine, Surgery, CCC, Critical Care, Rehabilitation, and Women’s & Children’s/ER is required;
  • Experience providing comprehensive transition management/discharge planning while utilizing an excellent knowledge of levels of care, applicable legislation, and community resources;
  • Demonstrated ability to organize workload and establish priorities effectively within a fast paced environment with a varied caseload;
  • Excellent communication skills, orally and written;
  • Demonstrated effective leadership and interpersonal skills;
  • Experience with program development and evaluation;

  • Must have a satisfactory performance record;
  • Demonstrated understanding of and commitment to Lakeridge Health’s comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-service in this area.
