An exciting job role has recently become available in Lancashire. They are looking for a new member for their Adult Hospital Discharge Team. What will your responsibilities be? You will evaluate the medical, social, and emotional needs of patients and develop individualized discharge plans in collaboration with the healthcare team and the patient’s family. You will work with healthcare providers, community agencies, and insurance companies to arrange for home care services or transfer to other facilities. You will provide counseling and emotional support to patients and their families during the discharge process. Additionally, you will educate patients and their families about available resources and support systems, and advocate for patients’ rights and needs to ensure they receive the best possible care.
