Purpose
Reporting to the Executive Director, the Social Worker is responsible for the development and implementation of the Social Work Program within the long-term care home that supports the “Colour It Your Way” resident led philosophy of care. The goal of the Social Work program is to ensure that residents receive a safe and person-centered plan of care that aligns with the Ministry of Long-Term Care (MLTC) act, regulations, and standards, and adheres to the professional accountabilities set by the Ontario College of Social Workers and Social Service Workers.
Social work services are designed to meet the psychosocial needs of residents, their families, and others involved in residents’ care and are delivered in a manner that ensures confidentiality and cultural competence in accordance with professional standards. Social services include, but are not limited to spiritual programs, palliative care support, individual or family therapy, emotional and behavioural support, wellness promotion and connections to community resources.
Responsibilities
General
- Supports, interprets, and incorporates the corporate mission, core values and “Colour It Your Way” resident-led philosophy of care, into all aspects of the provision of services.
- Assists and supports new resident/substitute decision maker with admission into the Home. Provides one-to-one counseling where necessary and may refer to external resources for further counseling for the purpose of facilitating the resident’s adjustment to the Home, supporting psychosocial well-being, and supporting quality of life dimensions.
- Provides counsel and therapy to assist residents and/or family members to develop skills to deal with and resolve their social and personal problems, and an ad hoc or planned intervention basis. May also develop Support Group services for residents and family.
- Provides education to residents and their family members.
- To function as a member of the multi-disciplinary team in planning overall resident care.
- Promotes a resident-centered care approach that engages and supports residents and family members with respect to living in the home, behavioural and life adjustment concerns, etc.
- Facilitate communication and collaboration between residents, their families, healthcare providers and other relevant stakeholders.
- Advocate for the residents by promoting the Residents’ Bill of Rights and addressing issues related to Powers of Attorney, Substitute Decision Makers, The Consent and Capacity Board, and the Public Guardian and Trustee.
Clinical & Quality Management
- Conduct psychosocial, cognitive, depression assessments.
- Complete suicide risk assessments, interventions and monitoring.
- Document clinical data, assessment findings, plans of care, therapeutic interventions, resident responses, in a timely and accurate manner.
- Planning, developing, implementing and evaluating individual social work services to meet the identified needs of residents and family members; ensuring those activities are aligned to the home’s strategic plan, mission and values.
- Leads the ethics committee for the home and is available to staff to consult with regarding ethical dilemmas within the home and works collaboratively with staff to resolve ethical dilemmas.
- Provides monthly reports on key indicators to the Executive Director/Designate.
- Participates in committees related to resident care and operation of the home as required.
- Attends the multi-disciplinary care conference with resident and family members as required by the Resident being reviewed.
- Supports internal resident transfers in collaboration with the resident/substitute decision maker, facility staff and other residents.
- Supports the End-of-Life Program by providing emotional, social, and practical support to residents and their families, ensuring their end-of-life wishes are respected and honoured.
- Provides orientation and education to the team members of the Home regarding the creation of a therapeutic environment; congenial atmosphere; healthy interpersonal relationships; enhancing staff understanding of the normal aging process and the Residents’ Bill of Rights; heightening insight into early recognition and intervention of psychosocial problems; and promoting referral of residents to the Social Work services.
Community Relations
- Works in conjunction with the Behaviour Supports Ontario (BSO) program.
- Liaises with Social Service Workers in the community for the purpose of transitioning a resident back to the community. Liaises with community Mental Health Care services, offering resources and assisting with the transition of residents who have mental health diagnosis.
Occupational Health & Safety
- Works in accordance with the organization’s Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act.
- Perform other related duties that may be assigned.
Working Conditions
Hours of work are based on a 35-hour week. Works in an accessible manner with frequent disruptions and distraction from residents, family members, visitors and employees that require assistance and information. May include evenings, nights and weekends as needed to support residents, families and employees.

- The leadership team, team members and caregivers, residents, Medical Director, attending physicians and nurse practitioner.

- Resident’s families and representatives, Ministry of Long-Term Care, Public Health, contract services providers, Ontario Health at Home and the general public.
Knowledge and Skill
- Bachelor’s or Master’s degree in Social Work from an accredited university program.
- Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Have at least two (2) years of relevant experience in social work or other related field, preferably in health care/long term care.
- All applicants must successfully complete the prescribed Vulnerable Sector check before their start date.
- MoCA Montreal-Cognitive Assessment trained and certified.
- Able to work independently and as part of a team.
- Demonstrated leadership, excellent communication, organizational and time management skills are required.
- Clinical competence and commitment to quality resident care.
- Experience with electronic health records (EHR) systems.
- Crisis intervention skills.
- Have a passion to promote person-centered care and work with the senior population.
- The ability to engage with a wide variety of people with a high level of professionalism, compassion and courtesy including culturally, socially, and economically diverse populations, seniors, persons with disabilities and professional colleagues.
- Ability to ensure confidentiality and privacy of resident information
- Commitment to excellence in professional practice and continuous professional learning.
- Maintain good work attendance and is flexible with time.
- Knowledge of the Ministry of Long-Term Care Fixing Long-Term Care Act, 2021, and Regulations.
- Previous use of computer software applications (e.g., Microsoft Office).
Impact of Error
Improper practice may result in a significant negative consequence to the well-being and quality of residential life.
The lack of compliance to relevant Acts and Policies may lead to the citing of unmet standards resulting in a loss of public trust.
Ineffective application of policies, programs and service delivery may result in lost confidence by the residents, staff and the public relating to the Home’s ability to provide appropriate care.
