Responsibilities

WHAT YOU’LL BE DOING

The Discharge Patient Flow Concierge Social Worker is responsible for supporting and facilitating discharge planning for patients across the Clinical Networks of the Mid North Coast Local Health District (MNCLHD) to assist patients and their families with the process of leaving hospital and transitioning back to their home or other care setting.
Whilst this is a network position, it will be imbedded as part of the social work team. The social work department values a collaborative and supportive culture that respects and nurtures the contributions of each team member. The team comes with knowledge and experience with Senior staff that can support learning and development. The Social work position will be offered mentoring, supervision and will be an active member of the social work department.
The position holder will do this by working in collaboration with the discharge planning and multidisciplinary teams across the network and proactively seek patients who are at risk of delayed discharge due to complex medical, social, or logistical issues early within their admission. The role will be supported by the discharge patient flow concierge administration officer and will work closely with interdisciplinary colleagues, social work team, and senior managers to develop effective and efficient Social Work services that addresses the psychosocial aspects of care for patient, as identified. The role will provide intervention including support, education and resources to people in the context of their bio-psycho-social situation, to assist in dealing with health events.

A TALENT POOL MAY BE CREATED FOR FUTURE ROLES AND WILL REMAIN VALID FOR 18 MONTHS.
