In this role you’ll be supporting renal patients and families through advanced psychosocial care, contributing to a multidisciplinary team, and leading in Renal Supportive Care.

• Providing counselling, advocacy & practical support
• Leading advanced care & end-of-life planning
• Delivering training to medical & allied health staff
• Supporting families through bereavement processes
• Collaborating across teams to ensure quality care
