Day Shift - 7.5 Hours (United States of America) Completes documentation including person centered treatment plans, progress notes, assessments, screening tools and results in a timely manner. Follows designated time frame identified in department policy regarding documentation. Completes high risk documentation including CSSRS and safety plan in accordance with department policy. Provides appropriate clinical services that are person centered and trauma informed. Services include intake assessments, individual and group therapy, collateral and family involvement, crisis intervention, patient/family psychoeducation. Provides and engages in collateral work with outside agencies, programs and services. Efficiently incorporates case management duties within work to enhance patient care. Actively works with the interdisciplinary team, including staff meeting and high risk meeting involvement. Communicates and collaborates with other professional staff. Actively engages in the supervision process/meetings and applies feedback to clinical work. Continues professional development by attending a minimum of 4 trainings per year with the intent of enhancing knowledge, sensitivity and self-awareness. Meets productivity expectation of daily visits set by the department Demonstrates effective time management including adhering to work shift schedule and management of patient appointments, scheduling and documentation. When clinically appropriate, completes toxicology screenings and necessary interventions based on UDS results. Adheres to monitoring and documentation expectations around the treatment of dually diagnosed individuals. Adheres to the department policy around the completion of outreach calls/letters, documentation of missed visits and gaps in treatment, all collateral contact and program consents Requirements: Master's degree required. NYS Registration/Certification as a Social Worker/LMHC required. Prior experience in a hospital or community health agency is preferred.
