Description
We are seeking an experienced Hospice Social Worker (MSW) to join our interdisciplinary team and provide compassionate psychosocial support to patients and families navigating end-of-life care. This role delivers coordinated care in the home setting and serves as an advocate, counselor, educator, and resource for patients and caregivers. AS A HOSPICE SOCIAL WORKER, YOU WILL: * Complete and document the Comprehensive Assessment within 5 days of hospice admission to evaluate psychosocial, emotional, financial, environmental, and community resource needs. * Develop and implement interventions that support patient goals and align with the individualized Plan of Care (POC). * Provide counseling and emotional support related to end-of-life care, grief, loss, family dynamics, and adjustment. * Assist patients and families with advance directives, funeral planning, discharge planning, and care transitions. * Coordinate community resources and connect families with available support services. * Participate as an active member of the Interdisciplinary Group (IDG) through collaboration and care planning. * Maintain timely and accurate clinical documentation. * Promote dignity, respect, patient rights, and compassionate service throughout the care experience. QUALIFICATIONS: ✔ Master’s Degree in Social Work (MSW) from an accredited program ✔ Current Social Worker License  ✔ Minimum 1 year of Hospice Social Work experience required ✔ Strong understanding of hospice philosophy and end-of-life care ✔ Valid Driver’s License and current Auto Insurance ✔ Current AHA CPR Certification * HomeCare HomeBase experience preferred
