JOB SUMMARY

The Social Worker Behavioral Health offers evidence-based personal, group, and family counseling and therapy. This helps cope with various behavioral health issues. It coordinates services like crisis intervention, client advocacy, referrals, prevention, education, discharge planning, and psychosocial assessments.

Responsibilities
  • Develops a counseling relationship with people, groups, and families. Uses clinical mental health and human development standards. Gains expertise in personal problems, defines goals, and creates action plans.
  • Conducts assessment interviews to identify problems and symptoms for diagnosis. Develops, implements, and evaluates comprehensive goal-oriented treatment plans.
  • Partners with psychiatrists and social workers to coordinate care and refer clients to help groups and treatment facilities. Involves the patient and family in developing and implementing plans.
  • Provides clinical counseling to people and significant others with highly diverse behavioral, psychological and/or sociological problems.
  • Conducts singular, family, and group therapy sessions as part of treatment plans. Ensures family members engage as much as possible in each program stage.
  • Provides crisis intervention, stabilization, and follow-up to establish that clients have received all appropriate services.
  • Partners with other healthcare team members to help the interdisciplinary care of patients and achievement of positive outcomes.
  • Participates in performance improvement initiatives to include data collection and review, development, and participation in continuing education programs.
  • Maintains all documentation from referral through discharge of person served.
