Description
As a member of the interdisciplinary health care team, the Social Worker will conduct psychosocial assessments and develop treatment plans that include counseling, education and patient and family interventions. The Social Worker will work collaboratively with the hospital team members, primary care, home care and other community providers to support patient centered care including seamless and timely transitions between hospital and community. Organizing and leading meetings with the multidisciplinary team to develop holistic care plans for patients with complex health and social needs will be a key responsibility of the Social Worker. 1. Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker 2. Master's Degree in Social Work from an accredited School of Social Work or Baccalaureate degree in Social Work with 3-5 years experience working in an acute medicine setting 3. Recent 3-5 years of demonstrated expertise and competence in assessment of social determinants of health and development of interventions to assist patients impacted by determinants of health 4. Recent 3-5 years demonstrated expertise and competence working with seniors who are frail and/or palliative and their families 5. Excellent interpersonal skills: including excellent oral and written communication skills, teamwork, consultation and teaching skills 6. Excellent skills in conflict resolution, organization, time management and problem solving 7. Demonstrated ability to work independently and collaboratively as part of an interdisciplinary team and with patients and families 8. Proven ability to work in a fast-paced, complex environment 9. Experience in providing crisis and bereavement counseling 10. Knowledge of relevant legislation governing health care delivery, including Health Care Consent Act, Public Hospitals Act, Substitute Decisions Act, etc. and of capacity issues 11. Demonstrated knowledge of child and family health and community support services 12. Demonstrated skills in the performance of a therapeutic role, including strong assessment, counseling, case management, crisis intervention and advocacy skills
