The Regional Neonatal Follow up Program of Boston Children?s Health Physicians (BCHP) has an immediate opening for a social worker (LCSW). Our program services infants/children and their families in the Lower Hudson Valley Region. Our patient population is comprised primarily of former neonatal intensive care unit (NICU) infants from NICUs all over the Lower Hudson Valley Region. However, the majority of our patients are those discharged from the Maria Fareri Children?s Hospital at Westchester Medical Center. Our team consists of four neonatologists, three family nurse practitioners, two clinical psychologists and a dedicated administrative team. We are seeking a hard-working professional who will confer and collaborate with our interdisciplinary team, families, and community agencies in the coordination of complex care for our patient population. The candidate should feel comfortable advocating for families within various resource systems and navigating service organizations in order to complete referrals. This will entail aiding in the arrangement of medical transportation when necessary, the coordination of outpatient medical appointments in addition to assisting with enrollment in local municipal early intervention programs for developmental therapeutic intervention. We look to identify a person who can assist in the transition of care for infants and their families from the NICU to home.
Currently, as a part of our inpatient and outpatient program, for NICU infants, we screen birthing parents for post-partum depression. When a positive screen occurs, that parent is then referred out of our program to a therapist for psychological mental health support. Our successful candidate will provide individual and/or family counseling for adults as this position will include mental health screening and teletherapy intervention for our patient?s caregivers.
Our goal as a program is to provide excellent medical clinical care, as well as being a resource for our families to lean on to support them socially and emotionally. Our inpatient team has worked tirelessly to provide these infants the opportunity to go home with their families, however, our work is not done there. It is our conviction that an integral part of these infants? road to both medical and neuro-developmental achievement requires caregivers who have the support socially and psychologically when necessary to continue our collective work. The person we are looking to engage and hire will be essential in achieving those goals.

Salary: $65,000-$80,000, negotiable based on experience
