Schedule Monday - Friday Days Benefits Day 1 Mileage reimbursement Key Responsibilities: • Responsible for the direct social work case management services to PACE participants. Provides social services support to include participant screening, case management, counseling, and referral. • Participate in the Interdisciplinary Team’s initial assessments, care planning and ongoing re-evaluations of participant’s case. • Maintain current written case management records, including periodic reassessments of program participants. • Refer participants to appropriate community agencies or facilities, act as a liaison with such organizations and as advocate for participants. • Consult with and advise staff members as to the relationship of social, emotional and cultural factors to health and medical care, and as to the availability of social services in the community. Knowledge & Skills: • Excellent communication; via phone, in person and email. • Requires individual therapy skills geared to geriatric patients, analytical thinking and problem solving abilities. • Attention to detail & ability to handle confidential information following HIPPA guidelines. • Must be flexible to work in other locations if needed. Required qualifications: • Master’s degree in Social Work from an accredited school. • Two years’ experience in a health related area desired. • One year of elder care experience required. • Valid Florida Drivers’ License with good record • Automobile Insurance • Clear level 2 background check Educational/Training Requirements: Masters Certificates, Licenses, and Registration: Must possess a valid Florida driver's license, current auto insurance, and reliable transportation. Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs. Experience Requirements: Two years experience in a health related area desired. One year of elder care experience is required. Refer participants to appropriate community agencies or facilities, act as a liaison with such organizations and as advocate for participants. Encourage effective relationships amongst staff geared to the building and maintenance of a cohesive team. Consult with and advise staff members as to the relationship of social, emotional and cultural factors to health and medical care, and as to the availability of social services in the community. Participate in program and policy development of the social work component of the program. Represent the company in a positive and professional manner at all times with others, e.g., co-workers, management, vendors, and customers.
