QUALIFICATIONS, SKILLS AND EXPERIENCE:

  • A Master’s degree in Social Work.
  • Registration with the Ontario College of Social Workers and Social Service Workers
  • At least 3 years of experience providing direct social work support to individuals from marginalized communities including those who experience homelessness, substance use, mental health, poverty and racialized communities
  • Experience working with homeless clients is preferred.
  • Working knowledge of the Ontario Mental Health Act, Healthcare Consent Act, Substitute Decisions Act and Community mental Health resources.
  • Extensive knowledge and experience working from a harm reduction perspective.
  • Clinical experience working in or collaborating with community-based agencies.
  • Knowledge of and commitment to client–centered philosophy and principles and practice of community service delivery and chronic disease management models.
  • Knowledge of community resources relevant to the provision of harm reduction, health promotion and illness prevention.
  • Knowledge and experience in the application of electronic medical records (EMR) preferred.
  • Proficiency in the use of the computer hardware and software, particularly in Microsoft Word, Excel, Outlook.
  • Ability to maintain professional practice regarding commitments to colleagues and clients, pertinent legislation, evidence-based standards of care and professional standards of practice.

Responsibilities

PURPOSE OF THE POSITION:

The Inner City Family Health Team is a multidisciplinary team which serves homeless clients with complex health needs. These include untreated chronic and complex multi-system diseases, an elevated risk of acute health emergencies, and serious and persistent mental health and substance use challenges. Services are provided by the team using a holistic, client-centred model of care with an emphasis on disease prevention, chronic disease management, harm reduction and health promotion.
The Social Worker collaborates with Inner City Family Physicians, Nurse Practitioners, our interdisciplinary team, and community to provide timely, effective and appropriate social work supports in accordance with FHT policies and professional standards to clients experiencing chronic diseases, complex multi-system diseases, and mental health and/or substance use challenges.

DUTIES & RESPONSIBILITIES

· Provides individual and group counseling for a range of mental health and/or addiction-related needs.
· Assists clients in identifying and working toward their goals using a holistic, client-centered harm reduction and Recovery model.
· Coordinate referrals and access to health care and community resources, provides psycho-education, advocacy and completes applications/forms in support of client goals (e.g., housing, medication coverage, CPP).
· Develops, leads, and/or evaluates group programs (e.g., cooking, gardening, softball).
· Collects accurate and comprehensive client-related information (including client monitoring/progress and assessments) and documents on the FHT’s clinical management system (OSCAR EMR) and other tools
· Consults and communicates effectively with internal and external providers to ensure cohesive, integrated care.
· Supports organizational goals, participates in quality improvement, and contributes to a positive team environment.
· Undertakes home visits and outreach services as required and appropriate.
· Consults and collaborates with external professionals involved in the care and support of the client.
· Encourages client involvement in health care decisions and in taking action for their own health.
· Contributes to effective reporting by ensuring that encounter forms and other statistics have been recorded, including OSCAR billing
· Contributes to the development and attainment of organizational goals, related objectives and performance measures.
· Maintains and develops professional competence through ongoing professional development and maintaining current discipline specific standards of practice.
· Stays current and aware of opportunities to implement new evidence-based methods of client assessment, treatment and programming.
· Maintains awareness and understanding of current Mental Health legislation.
· Performs additional administrative tasks and other duties as assigned.
