Job/Position Description Social Worker Job Title: Social Worker Dept: Clinical Reports To: Patient Care Manager Supervisory Responsibilities: Yes X No FLSA Status: Exempt Non-Exempt Position Summary Primary function is to provide coordinated care in the home or facility to patients of all age groups and perform psychosocial assessments, analysis, counseling and referrals to meet the needs of the patient/family. QUALIFICATIONS AND EDUCATION REQUIREMENTS Bachelors or Master’s Degree in Social Work from a State accredited program. If employee has a Bachelor's degree and is hired after 12/02/08, must be supervised by a Master's of Social Work (§418.114). Current State license required in accordance with State regulations Current State Drivers License One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred Strong critical thinking, organizational, interpersonal, and communication skills Must be highly motivated, self-directed, flexible, and able to work well and participate in a multidisciplinary group setting Ability to work autonomously Experience in EMR a plus Working knowledge of community resources Must have reliable transportation and provide ongoing valid and current auto liability insurance ESSENTIAL DUTIES: Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals Use effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes Meet mandatory continuing education requirements of the agency/licensing board Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements Attend Inter-Disciplinary Group (IDG) meeting, participate in the patient care planning process and collaborates with IDG to promote coordination of patient care Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues Facilitate community resources based on patient/family needs Accept clinical assignments that are consistent with education and competence to meet the needs of the patients Perform on-call responsibilities and provide on-call service to patients/families as assigned In EMR/HCHB, all visits are to be completed and synced same day Time Tracker should be reviewed daily and Non-Visit Activity (NVA) expected to be entered when applicable Abide by and demonstrate the company Mission and 12 Heart’s Desires through both behavior and job performance on a day-to-day basis Participate in all mandatory staff in-services and meetings Maintain compliance with internal annual education requirements (Heart to Heart University/Relias) Timely submission of all documentation Conduct all business activities in a professional and ethical manner Performance of other duties as required Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures WORKING CONDITIONS The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Works in patients home in various conditions; possible exposure to blood and bodily fluids and to infectious diseases; ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency call. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient/family individualized psycho social needs. Requires hand-eye coordination and manual dexterity. Specific vision abilities required by this job include close vision and the ability to adjust focus. Prolonged standing, walking and sitting required; requires working under some stressful conditions to meet deadlines and patient/family needs; and to make quick decisions and resource acquisition; meet patient/family psychosocial needs.
