SUMMARY:

The Essex County Nurse Practitioner – Led Clinic (ECNPLC) provides primary health care services to the residents of Essex, Amherstburg, Kingsville and Windsor.
The Registered Social Worker works to their full scope of practice within an interdisciplinary team. They collaborate with all team members and report to the Administration Manager, and Executive Director. The Social Worker will provide individual counselling, assessment, advocacy, and referral services to patients. Social Workers must uphold the standards outlined in the Ontario College of Social Workers and Social Service Workers, Code of Ethics and Standards of Practice and hold a license to practice as a professional social worker.

QUALIFICATIONS

  • Masters degree in Social Work from a recognized university preferred.
  • Current membership in the Ontario College of Social Workers and Social Service Workers.
  • At least one year of counselling/therapy experience preferably in a community setting.
  • Proficiencies in assessment, psychotherapy and psychological counselling techniques for working with individuals, couples, families and communities.
  • Experience in psychotherapy is considered an asset.
  • Ability to work flexible hours and to travel as needed.
  • Demonstrated ability to work effectively in a team environment.
  • Proficiency in the use of computers and various software applications.
  • Excellent written and oral communication.
  • Excellent organization skills.
  • Current registration with the Canadian Social Workers Association recommended.
  • Current proof of professional liability insurance.
  • Current police clearance including vulnerable check.
  • Second language would be an asset.

Responsibilities
  • Assessment of psychosocial problems.
  • Develops and discusses individualized care plan with the client, family or care giver based on Social Work best practices and in collaboration with Nurse Practitioners and other health care professionals where appropriate.
  • Provides education related to the prevention of mental health problems.
  • Facilitate coordinated access to and linkages with services in areas such as housing, crisis intervention, treatment, health promotion, education, prevention, other supports and resources.
  • Evaluate achievement of patient goals.
  • Assist patients in personal effectiveness: problem -solving, decision making, communication, interpersonal skills, goal setting and time management.
  • Assist patients in community integration: use of transit, social/recreational, peer support and other services as needed.
  • Assist patients in health and wellness by supporting a clinical plan of care including medication, appointments, personal care, household management, healthy choices and lifestyle.
  • Advocate for client’s civil and legal rights.
  • Documents appropriately via electronic medical records.
  • Collaborates with other health care providers regarding after care plan.
  • Protects confidential and sensitive information related to patients.
  • Promotes awareness of ECNPLC services and programs.
  • Provides leadership and mentorship to Social Work students who may use ECNPLC as a clinical placement.
  • Participates in the education of other health professional students.
  • Participates in the development planning and evaluation of treatment, education, counselling and health promotion activities.
  • Stays current and aware of opportunities to implement new, evidenced-based methods of client assessment, treatment and programming.
  • The social worker will participate in group work at ECNPLC sites and within the community
  • The social worker will engage in appropriate training as appropriate to service needs including One Team Recovery substance use program training
