We are currently seeking a dedicated and skilled Social Worker to join our team in providing comprehensive services to older adults with severe mental illness, with the goal of promoting recovery and successful discharge to the community. This position is responsible for planning, organizing, and implementing individualized treatment goals using sound therapeutic approaches, conducting ongoing assessments, and delivering individual, group, and family supportive counseling. As an active member of the interdisciplinary treatment team, the Social Worker will maintain strong communication and collaboration with patients, their primary support systems, referring agencies, and community resources. The ideal candidate will possess the ability to develop and implement treatment plans, gather and interpret medical and psychosocial data, and communicate effectively both orally and in writing. A thorough knowledge of the NASW Code of Ethics, working familiarity with community resources, adaptability in a fast-paced environment, and the ability to work independently while contributing as a valued team member are essential to success in this role.
