Description
Pomeroy Living is seeking a compassionate and experienced Social Worker to join our team. As a key member of our interdisciplinary care team, you will play a vital role in providing psychosocial support and advocacy to our residents.   Responsibilities: * Conduct comprehensive psychosocial assessments of residents, including identifying social, emotional, and spiritual needs. * Develop and implement individualized care plans that address resident needs and support their overall well-being. * Provide counseling and support to residents and their families on a variety of issues, such as grief and loss, adjustment to care, and end-of-life care. * Advocate for resident rights and ensure their quality of life. * Assist with discharge planning and coordinate community resources. * Collaborate with the interdisciplinary care team, including physicians, nurses, therapists, and other staff. * Maintain accurate and timely documentation in accordance with regulatory requirements. * Participate in facility-wide quality improvement initiatives. * Conduct resident and family meetings as needed. * Provide support and guidance to staff on psychosocial issues. Qualifications: * Minimum of Bachelor’s Degree in Social Work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology, Master’s Degree preferred.  One year supervised social work experience working directly with individuals in a long-term or sub-acute care rehabilitation. * Current Social Work license in the state of Michigan preferred * Excellent communication, interpersonal, and documentation skills. * Strong assessment, problem-solving, and critical thinking skills. * Ability to work independently and as part of a multidisciplinary team. * Passion for working with older adults and a commitment to providing high-quality care.  #PM25 Benefits: * Paid Time Off - start accruing day one of orientation * Paid Holidays  * Access up to 50% of your earned pay DAILY! * Flexible scheduling * Company paid Life insurance for FT staff * Priority Health Medical, Delta Dental and Vision Insurance * 401(k) retirement plan * Career advancement opportunities Join our team at Pomeroy Living and make a difference in the lives of our residents!
