Description About the Role: Touchstone is looking for a compassionate and experienced PRN Medical Social Worker (MSW) to support patients and families in their homes across San Antonio. In this dual Home Health and Hospice role, you’ll be part of a caring, highly collaborative team that values strong leadership, flexible scheduling, and the importance of work-life balance. Key Responsibilities: Provide in-home psychosocial support aligned with patients’ care plans. Help patients and families navigate emotional, financial, and community resources. Serve as a key liaison between patients, families, physicians, and the care team. Attend patient care conferences and contribute to care planning. Safeguard patient confidentiality and uphold HIPAA standards. Assist with discharge planning and transitions of care. Maintain accurate documentation and timely progress notes. Participate in ongoing education and quality improvement efforts. Requirements What We’re Looking For: Master's degree in Social Work (MSW) from an accredited program. At least 1 year of experience in a healthcare setting (home care or hospice preferred). Excellent communication and relationship-building skills. Ability to work independently and collaboratively across care teams. Strong understanding of Medicare Conditions of Participation. Current driver's license and reliable transportation. Flexibility to support patients on a PRN/as-needed basis. A deep commitment to patient-centered care and dignity at end-of-life.
